The Network is working with the Toronto Stroke Networks to improve flow for hip fracture and stroke patients with complex needs.
Earlier this year, the Networks convened a think tank of front line staff and managers from across the five GTA LHINs. One outcome was a detailed complexity framework that identifies the complex psychosocial, functional and medical issues that contribute to extended hospital stays for these patients.
The gathering also identified a broad range of ideas to begin to address these issues. Suggestions included developing strategies to implement the framework and creating alternative care pathways for hip fracture patients with complex needs.
The focus now shifts to developing demonstration projects that optimize patient flow. The Networks plan to establish a cross-sector steering committee to facilitate this work.
