Access to information is the first step to ensuring access to services.
Rehab Finder provides a searchable database of rehabilitation programs/clinics/services
provided by hospitals and Community Care Access Centres (CCACs)
across the Greater Toronto Area.
Rehab programs/clinics/services can be searched by:
- patient population (including age)
- service setting
- Local Health Integration Network
- organization
- special needs
Information provided includes:
- description of the programs/clinics/services
- contact details
- application process and forms
- information about waiting lists
The
database lists:
- inpatient and outpatient multi-disciplinary rehab programs
- single services/specialty clinics
- fee-for-service programs/clinics/services located within
hospitals
- rehabilitation provided in complex continuing care
beds
