Complex Hip Fracture/Stroke Patients

The Network is working with the Toronto Stroke Networks to improve how complex hip fracture and stroke patients move through their hospital stay — from acute care, through rehabilitation and back to the community. 

These patients experience long Alternate Level of Care (ALC) stays in hospital for a variety of reasons, including multiple medical conditions, time needed for recovery and lack of support systems at home or in the community. These factors have been captured in a complexity framework

Work is underway to develop demonstration projects that improve the flow of these patients through the system.
