Discharge Planning Guidelines

The Network's Discharge Planning Guidelines for Inpatient Rehabilitation set out guiding principles, standards and discharge readiness criteria for inpatient high tolerance and low tolerance rehab programs to promote consistency and best practices in discharge planning.

The guidelines incorporate the Provincial Alternate Level of Care (ALC) Definition in use in all acute and post-acute hospitals and supplement the provincial ALC toolkit.

The development of these guidelines has been supported by funding from the Toronto Central LHIN.
