As care delivery shifts to Ontario Health Teams (OHTs), the Network is helping them include rehabilitative care in their services.

Rehabilitative Care: An Essential Component of Connected Care is a brief guide for OHT applicants developed by the Network and Rehabilitative Care Alliance. It provides an overview of how rehabilitative care services across the care continuum help patients regain health and quality of life. The guide also demonstrates how providing rehabilitative services can help OHTs deliver on the quadruple aim.

As part of its new strategic plan, the Network is focused on identifying leading practices and innovative models of rehabilitative care. The Network plans to work with GTA OHTs to customize these practices and models for the OHT context.
