Network's rehab/CCC forum for acute care hospitals a sellout

In October 2012 the GTA Rehab Network hosted a "sold out" forum on rehab/CCC programs for acute care hospitals that attracted over 80 healthcare professionals.

The forum provided referring hospitals with updates on available inpatient rehab/CCC programs and an opportunity to clarify and improve the inpatient referral process, particularly for patients with complex needs.

Representatives from each rehab/CCC hospital gave a 10-minute overview on:

  • Programs that are unique to their hospital for "medical/geriatric populations"
  • Differentiating criteria for medically complex populations, e.g. referring hip fracture patients to MSK rehab vs. geratric rehab vs. slow stream rehab
  • Specific referral and previous medical history information that rehab/CCC programs require for their admissions decisions

Attendees found the forum to be "an excellent educational session to promote awareness of what is available for patients" and "a great opportunity for networking". Many requested a similar forum on outpatient rehab, particularly for patients following total joint replacement. Stay tuned for further details.
