Quick Reference Referral Guide

Access to information is the first step to ensuring access to services.

The Quick Reference Referral Guide to Inpatient Rehab, Complex Continuing Care and Palliative Care Programs in the GTA is a printable, easy-to-read overview of discharge options to facilitate discharge planning discussions/decisions between the care team and patient/family.

The Referral Guide provides a brief summary of each program for the purpose of easily locating an appropriate bed for a specific client, including:

• number of beds on the unit
• types of patients accepted
• admission and exclusion criteria, and
• special needs accommodated.

The Referral Guide lists publicly-funded programs offered by members of the GTA Rehab Network that accept external referrals. For a complete listing of programs and services, visit Rehab Finder, our searchable database of rehab programs/clinics/services.
