Referral Guidelines

The smooth and efficient transfer of patients is critical to ensuring individuals have access to appropriate rehabilitation care when they need it.

The Network's Inpatient Rehab/LTLD Referral Guidelines for acute care and rehab hospitals are designed to optimize referrals for both regular and low tolerance long duration (LTLD) inpatient rehab.

The guidelines provide criteria to help determine when a patient is:


  • a candidate for inpatient rehab
  • medically stable
  • rehab ready
  • appropriate for ALC designation

and benchmarks for:


  • timing of referrals relative to rehab readiness
  • number of referrals to be submitted
  • timing of responses to referrals
  • response categories to use when responding to rehab applications.
