Referral Information Letter for Patients
The Network has developed a new standardized information letter for patients in acute care to support the rehabilitation referral process.
Staff in acute care hospitals will provide the letter to patients and families during discharge planning discussions if the patient is being referred to inpatient rehab or low tolerance long duration (LTLD) rehab programs in a rehab/CCC hospital.
The standardized letter ensures that patients and families receive:
- a simple and clear introduction to rehabilitation to help them understand and prepare for this next phase of care
- information about rehabilitation earlier in their recovery, and
- the same information, regardless of the hospital they are in.
The letter was developed with input from directors, managers and front-line social workers/discharge planners in acute care and rehab/CCC hospitals within the Toronto area.
