The GTA Rehab Network is a recognized leader in improving the planning and delivery of rehabilitative care. Our success is due to strong collaborations with members, clinical experts and the GTA LHINs. Through these efforts, we support our members and the GTA LHINs as they work to advance best practices and enhance system efficiencies.
Current collaborations include:
- Stroke rehabilitation: The Network works closely with the three Toronto-area stroke networks to align care with best practices and improve utilization of stroke services. This includes participation on the e-stroke executive committee and the acute care and rehab stroke flow task groups. The Network also collects and analyzes performance indicators across outpatient stroke rehab programs.
- ALC: The Network is providing support to members in the Central LHIN to help them identify factors contributing to longer ALC stays. The Network has expertise in collecting and analyzing ALC data to improve referral processes and patient transitions.
- Support for Rehabilitative Care Alliance initiatives: The Network sits on Rehabilitative Care Alliance task groups to help them draw on earlier work done by the Network and its members. This serves to strengthen and advance the provincial efforts.
- Performance measurement of TJR outpatient rehab: RM&R is now supporting electronic referrals to outpatient rehabilitation for total joint replacements. The Network is working with RM&R to ensure the system captures data elements that support performance measurement.
